How we communicate with you via text messages

1. How You Opt-In

By providing your mobile number, you expressly consent to receive SMS messages from Premier Bariatric Institute regarding:

  • Appointment confirmations & reminders
  • Follow-ups & important clinical updates
  • Occasional educational messages about your care

To opt-in, you may submit your phone number through our Intake Form during your registration process or provide verbal consent during your in-office visit, which will be documented in your medical record.

2. Opt-Out Anytime

Not interested in receiving texts? You can opt out anytime:

  • Reply STOP to any message to unsubscribe.
  • Call us at (773) 365-1300 to request removal.

Once opted out, you won't receive further messages unless you reply START to re-subscribe.

3. Message Frequency & Rates

Message frequency varies based on your appointment schedule and care needs. We typically send 1-4 messages per month. Standard messaging & data rates may apply based on your carrier plan. Message and data rates are determined by your wireless provider and are not set by Premier Bariatric Institute.
