Notice Regarding Recording of Virtual Sessions
Premier Bariatric Institute may record certain virtual consultation sessions to support your care. Recording happens only with your permission. This page explains how we ask for that permission, what recordings are used for, and how they are protected.
We ask before we record
No session is recorded automatically. When you schedule a virtual visit, we ask whether you agree to recording. At the start of any session we intend to record, your provider will confirm your consent verbally before recording begins. If you have not agreed, the session is not recorded.
You may decline or change your mind
Declining to be recorded has no effect on the care and services you receive. You may also withdraw your consent at any point, including during a session or after it has ended. To do so, tell your provider directly or contact us at info@premierbariatricinstitute.com or 773‑365‑1300. Where a recording is not required as part of your medical record, we will delete it upon request.
How recordings are protected
Recordings are protected health information under HIPAA. They are stored securely, and access is limited to authorized members of our workforce. Recordings may be transcribed or reviewed using secure service providers that operate under business associate agreements with our practice. Recordings that form part of your medical record are retained consistent with our medical record retention policy; all others are deleted once their internal review purpose is complete.
Why we record
Clinical record keeping. A recording preserves an accurate account of your visit, which supports continuity of care and informed decisions about your treatment.
Supervision. When a provider is working under supervision, recordings allow a supervising physician to review the encounter and provide feedback.
Quality assurance. We review recordings internally to confirm adherence to clinical best practices and to improve the quality of care we deliver.
Training and education. Recordings may be used to train healthcare professionals within our practice. Any use of an identifiable recording outside our practice requires your separate written authorization; otherwise, identifying information is removed first.
