Your consultation slot is reserved, not yet confirmed. Completing your intake form is what confirms it.

It takes about 20 minutes and can't be saved partway, so set the time aside and complete it in one sitting. Have your insurance card handy if you plan to use insurance.

Your slot is held until Wednesday, September 16 at 10:00 AM. If the form isn't in by then, the slot is released to another patient.

The moment your form is in, we send your confirmation with the visit details and a calendar invite. We've also texted and emailed you this link.
